The Governments Innovative Models of Care (IMOC) Round 4 Grant Opportunity GO6283 is now open and applications close 2pm ASET Monday 7 August 2023. The grant opportunity guidelines are available on Grant Connect www.grants.gov.au by searching GO6283.

 

The IMOC program funds the trial and evaluation of locally co-designed, innovative and multidisciplinary models of primary care in Modified Monash (MM) Model 3-7 areas. The objectives of the program are to:  

  • trial a range of locally co-designed innovative multidisciplinary models of primary care, to address chronic workforce shortages and access issues, with trials spread across a range of rural and remote geographic contexts and involving a wide range of stakeholders; 
  • generate robust evidence about which models work, where they work best and assess the financial sustainability; and 
  • learn from funded trials and share learnings that will allow other communities to apply suitable innovative models locally to improve community health services access.
